How To Use ECG 7 Step Ruler

Step1: Measure Your Heart Rate

Find one of the tall spikes (R waves) that lines up with the “start” marker on the ruler.Record the heart rate two R-waves.In this illustration, the heart rate is about 130bpm.

Step 2: Check If Your Heartbeats Are Regular

Place the ruler on your ECG graph.Look at the spaces between the tall spikes (R waves).If the spaces are roughly the same, your heartbeat is regular. If not, it might be irregular.

Step 3: Determine the Electrical Axis

Check the mostly waveform in Lead I、Lead II and Lead III.Use the simple rule on the ruler to determine the presence of a cardiac axis shift.

Lead I ↑, Lead II ↑, Lead III ↑: Normal axisLead I ↑, Lead II ↑, Lead III ↓: Left axis deviationLead I ↓, Lead II ↑, Lead III ↑: Right axis deviationLead I and Lead II both ↓: Extreme axis deviation

Important Note：If the P waves are missing or look unusual, it might be worth checking with a doctor.

Step 4: Measure The Time From P Wave To Tall Spike (PR Interval)

Check if there is a small wave (P wave) before each tall spike (R wave).Use the ruler to measure the time between the start of the small wave (P wave) and the start of the tall spike (R wave).The normal range is 0.12 to 0.20 seconds (marked on the ruler)

Step 5: Check The Width Of The Tall Spikes (QRS Width)

Use the ruler to measure the width of the tall spikes.Normal width is 0.08 to 0.12 seconds (marked on the ruler).If the spikes are wider, it could indicate an issue.

Step 6: Look At The Flat Line After The Spikes (ST Segment)

Step 7: Assess For Any Abnormal T Waves

Place horizontal black along the isoelectric line.This line should stay flat and not dip or rise.If the T-wave shape appears to be a pattern on the scale, it may indicate a heart problem. Seek medical attention

V1-V6 leads show heart activity in specific areas; please consult a doctor for interpretation.The QT interval measures the time for ventricular activity.Normal QTc is under 0.35 to0.46 seconds for men and 0.35 to 0.44 seconds for women.Abnormal values may indicate heart rhythm issues—consult a doctor for interpretation.
